add physicalpath to be able to create Application under Default Web Site
fix when appcmd.exe app runs as set make sure the parameter is app.name instead of site.name
when set is run instead of add use /[path='/'].physicalpath
set previous fix for set and add just for app resource
account for iis_app with and without physicalpath attribute
